x = the solution, what we want to find
120 grams of 20% fat = 0.2 * 120
What we want is mixture of 18%
So....
0.1x + 0.2 * 120 = 0.18 (x + 120)
Solve:
0.1x + 24 = 0.18 (x +120)
0.1x + 24 = 0.18x + 21.6
24 = 0.08x + 21.6
x = 30
Barista should add 30 grams of cream in order to obtain 18% fat mixture.
