Slope intercept form is y=mx + b

The objective is to get the  y  variable all by itself.

For example, 2x+4y=12

First you would subtract both sides by 2x, giving

4y=-2x+12 (the original 12 was positive so give it a plus sign)

Then divide both sides by 4 to isolate that y. That would lead to

y=-2x/4 +3

Which simplifies to

y= -x/2 +3

This is one example. But remember, the way to put the equation into slope intercept form is to isolate the y like I've done above!
