The amplitude is the coefficient of the cosine term or A = 1/2
The frequency is the coefficient of the t term in the argument to the cosine divided by 2pi, or freq = 8/(2pi) = 4/pi.
I guess by midpoint line you mean the line that corresponds to the cosine term being equal to 0 and this is given by the offset that's added to the cosine term, i.e.
y = 2
