Here is POINT - slope form

 

y - -1   = -5/4 ( x - - 4)    re-arrange to slope intercept form

y +1 = - 5/4x  - 5

y = - 5/4 x -6

slope = (4+1)/(4-5) = -5

 

so Point Slope form <=> y+1 = -5(x+4)

 

so slope intercept <=> y = -5x -21
