If you substitute 4 for x into each of the possibilities, only (d) gives a y-value of -5; so, choose (d).

 

Generally speaking, if the vertex occurs at (h,k), the formula will be:  y  =  a(x - h)2 + k.

The term a is an "expansion factor".
