Standard form is :

Ax + By = C      subtract Ax   from both sides

By  = -Ax + C          divide both sides by B

y = (-A/B) x + C/B         now (-A/B)  = the slope = m  ......    and  (C/B )   = b = the y intercept .......so we have

y = mx  + b

And this is slope-intercept form
