How do you put an index in the root sign?

there  are several ways I think

 

I'd press the [LaTex Formula]  button and write something like     \sqrt[3]{15}   and then press OK

 

$$\sqrt[3]{15}$$             This is what I got.

 

Does that help?  Have a play with it you can do a lot ot things with LaTex.
