y-6=-3(x+2)

Standard form is when y=ax+b. To do that distribute -3 to (x+2) and get y-6=-3x-6. After that make sure you have y alone, so add 6 to both sides to get rid of " y (minus six)". You should end up with y=-3x-12 which is standard form (where -3 is slope and -12 is offset)
