Let's find the slope first.

 

6-4/9-3

 

2/6, or 1/3.

 

So, y=1/3x+b

 

Now, if x=0, then we'll find the y intercept(b), and vice-versa for the x intercept.(y=0 for x intercept[NOT NEEDED FOR THIS PROBLEM])

 

Let's substitute (3,4) into the problem.

 

4=1/3*3+b

 

4=b+1

 

Subtract 1 from both sides.

 

b=3.

 

So, the slope-intercept form is y=1/3x+3
