r =  12sqrt(2)

 

θ =  arctan ( 12/12)  = arctan (1)  = pi/4

 

So

 

(12sqrt 2 * cos (pi/4)  , 12 sqrt (2) , sin (pi/4) )

 

Also .... add pi to pi/4 = 5pi/4      make  r  negative

 

(-12sqrt (2) cos ( 5pi/4)  , -12sqrt (2) sin (5pi.4) )
